GERD treatment has two jobs: heal and prevent
Medical treatment does the first job and your habits do the second. GERD is acid repeatedly forced past the valve at the top of the stomach; treatment from your doctor calms that acid so the esophagus can heal, but it doesn't change how often acid gets pushed up. Only your habits do that. Treat GERD in this order and it stays treated.
Phase 1 β Get a real diagnosis
If heartburn happens twice a week or more, or you have any alarm symptom, see a doctor. Often that means an upper endoscopy to look for esophagitis, mucosal breaks, strictures or Barrett's esophagus. The result decides how aggressive Phase 2 needs to be. Understand the code on your report β
Phase 2 β Heal the lining (with your doctor)
An inflamed or broken lining needs time without being burned. What that takes - and for how long - depends on what the endoscopy found, your history and any other medicines you use, so it is a decision for your doctor, not a web page. Follow their plan exactly and don't stop or change it on your own. What medicine can and can't do β
Phase 3 β Change how you eat (the part that keeps it gone)
- Eat slowly and chew thoroughly. 20+ minutes per meal. Less stretch on the stomach, less pressure on the valve. This was the change that ended my GERD.
- Smaller, more frequent meals. Never eat to "full".
- Nothing within 3 hours of lying down. Night-time reflux does the most damage.
- Remove trigger foods β fatty, fried, spicy, tomato, citrus, chocolate, mint, coffee, alcohol, fizzy drinks β then reintroduce one at a time. Full GERD diet β
- Raise the head of the bed 15β20 cm and sleep on your left side.
- Lose excess abdominal weight, loosen tight clothing, stop smoking.
My experience: my doctor's treatment healed a severe mucosal break in about two months. But the other instruction β "eat slowly" β was the one I kept failing, and without it the reflux would have come straight back. I built an app to pace my meals. Phase 4 β Step down the medicine
Once the lining has healed and Phase 3 is habit, go back to your doctor and review the plan together. Whether treatment is reduced, changed or continued depends on what was found and how you're doing - never change it on your own. In my case, once the habits held, I no longer needed anything.
When GERD treatment needs to escalate
- Symptoms persist after 8 weeks of treatment plus lifestyle changes β pH monitoring and manometry to confirm the diagnosis and rule out other causes.
- Large hiatal hernia or reflux that returns every time treatment stops β anti-reflux surgery (Nissen fundoplication, LINX magnetic ring) or endoscopic procedures (TIF).
- Stricture β endoscopic dilation.
- Barrett's esophagus β surveillance endoscopies; ablation if dysplasia is found.
How long does GERD treatment take?
- Days: symptom relief once treatment and meal changes start
- 4β8 weeks: esophagitis heals
- 2β3 months: new eating habits become automatic
- Then: plan reviewed with your doctor; GERD stays in remission as long as the habits do
Seek urgent care for chest pain with sweating, breathlessness or pain spreading to the arm or jaw; vomiting blood; black stools. See your doctor soon for painful or difficult swallowing, food sticking, unexplained weight loss, or symptoms not improving after two weeks of treatment.
