GERD pain is reflux pain that has stopped being occasional
The sensation is the same as ordinary acid reflux pain โ burning or pressure behind the breastbone, sometimes climbing toward the throat. The difference is the pattern. In GERD, acid reaches the esophagus so often that the lining becomes inflamed, and inflamed tissue hurts between meals, not just after them. That's when "heartburn" turns into "my stomach hurts every day".
Where GERD hurts
Burning or heavy pressure behind the breastbone, worse after meals, bending, or lying flat. The classic heartburn location.
A gnawing ache or soreness just below the ribs (epigastric pain), often with bloating. This was my daily pain.
Rawness, soreness, or pain on swallowing (odynophagia) as acid reaches the upper esophagus and inflames it.
A dull ache between the shoulder blades. The esophagus and upper back share nerve pathways, so the brain misplaces the signal.
How GERD pain differs from ordinary heartburn
- Timing: heartburn follows a specific meal; GERD pain follows most meals and often shows up at night or on waking.
- Duration: heartburn clears in an hour or two; GERD pain lingers or becomes a background ache.
- Swallowing: GERD can make swallowing painful or make food feel stuck โ heartburn doesn't.
- Relief: heartburn eases and stays away; with GERD any relief is short and the pain returns the same day.
My experience: the first years were a burn after lunch. Then the burn became a constant ache under my ribs โ no meal felt safe, and I planned my day around my stomach. GERD chest pain vs heart attack
GERD chest pain is one of the most common reasons people go to emergency rooms โ and that's the right call when you're unsure. Typical differences:
- GERD: burning; after meals or when lying down; eases when sitting up; sour taste, burping.
- Heart: pressure, squeezing, heaviness; brought on by exertion or stress; spreads to arm, neck, jaw or back; sweating, nausea, breathlessness; position doesn't help.
If you cannot tell, treat it as your heart and call emergency services. Also see a doctor soon if GERD pain comes with painful or difficult swallowing, weight loss, vomiting, or black stools โ those need an endoscopy.
Relieving GERD pain
Right now
- Sit or stand upright; loosen your waistband.
- Sip water or chew sugar-free gum to clear and neutralise acid.
- Don't lie down for 2-3 hours; if it's bedtime, raise the head of the bed.
